Contact forms

You may use our contact forms, new case form or new case inquiry form to reach us in order to inquire regarding our services or to set up an appointment. We do respond to email inquiries and you agree that our response is requested and desired communication from us. The contact form has a field that you can check in order to receive occasional emails from us regarding new services or request to be added to our no more than twice monthly newsletter. If you check the box to receive these this is considered consent to receive these emails from us. You may opt out at any time.

Akismet is enabled on the site, the contact form submission data — IP address, user agent, name, email address, website, and message — is submitted to the Akismet service (also owned by Automattic) for the sole purpose of spam checking. The actual submission data is stored in the database of the site on which it was submitted and is emailed directly to the owner of the form (i.e. the site author who published the page on which the contact form resides). This email will include the submitter’s IP address, timestamp, name, email address, website, and message.

Data Synced: Post and post meta data associated with a user’s contact form submission. Akismet is enabled on the site, the IP address and user agent originally submitted with the comment are synced, as well, as they are stored in post meta.
